In accordance with the General Data Protection Regulation (GDPR) I take your privacy seriously and the following explains what data I keep and why I need to do so.
When you complete the Contact form on this website (www.inclusivetherapypractice.co.uk) I (Jacinta Deacon) receive an email at inclusivetherapypractice@gmail.com with the information you have given. I use this information contact you regarding your enquiry. This information is not used for marketing activities and is only used only for the purpose for which it was given.
If we begin to work together this information will be used to contact you, primarily regarding our appointments. In our first session, I will ask you to complete a personal details form containing your name, address, date of birth, contact information and the contact information of your GP. This information, along with a copy of our counselling agreement, is stored securely and confidentially in a locked cabinet. It is not shared with other people without your consent unless it is for medical or legal reasons.
I make brief notes after each session, these are anonymised and do not contain any personal details that could be used to identify you. These notes are kept separately from our counselling agreement and your personal information form. Your personal details form is confidentially destroyed when our work together ends, and in line with current industry guidelines, your anonymised notes are kept in a locked secure cabinet and are confidentially destroyed after 7 years.
As the DPO for Inclusive Therapy Practice, I want to ensure that any data I hold about you is accurate and therefore you have the right to request a copy of that information. To make this request please email me at inclusivetherapypractice@gmail.com
You have the right to ask for a copy of your personal information. You also have the right to ask me to amend or change any incorrect information about you. You have the right to ask me to erase any information that I hold about you. This includes your personal information that is no longer relevant to original purposes, or if you wish to withdraw consent. In all cases and when considering such requests, these rights are obligatory unless it’s information that I have a legal obligation to retain.
